Nonionic contrast neurotoxicity after coronary angiography mimicking subarachnoid hemorrhage.

چکیده
Introduction The incidence of neurotoxic events due to intravascular application of iodinated x-ray contrast media (CM) has decreased markedly since the introduction of nonionic CM [1]. Severe neurotoxic complications are accompanied by disruption of the blood-brain barrier and CM extravasation [1–6]. Coronary angiography involving only nonionic CM has been reported to cause transient cortical blindness, confusion and amnesia, but no focal deficits [3, 4], whereas coronary angiography involving ionic CM was followed by more severe complications including seizures, motor and speech deficits [5, 6]. Here we report on a patient who developed transient hemiparesis, aphasia and confusion due to neurotoxicity of nonionic CM after coronary angiography, and whose CT findings mimicked subarachnoid hemorrhage (SAH).

Subarachnoid Hemorrhage Mimicking Leakage of Contrast Media After Coronary Angiography
We report a patient who developed subarachnoid hemorrhage (SAH) just after coronary angiography (CAG) with non-ionic contrast media (CM) and minimal dose of heparin. The 55-year-old man had a history of acute ST elevation myocardial infarction that had been treated with primary percutaneous coronary intervention and was admitted for a follow-up CAG. The CAG was performed by the transradial appr...
متن کاملTransient cortical blindness: a benign but devastating complication after coronary angiography and graft study.
Transient cortical blindness after coronary angiography and bypass graft is a very rare complication. In this report we present the case of a 63-year-old man who developed transient cortical blindness within 30 minutes of coronary angioplasty and graft study, but subsequently recovered within 72 hours without any neurological deficit. A plain computed tomography brain scan showed bilateral symm...
متن کاملSubarachnoid hemorrhage mimicking ST-segment elevation myocardial infarction after return of spontaneous circulation
Electrocardiogram changes in subarachnoid hemorrhage (SAH) have been described as ST-T changes that mimic acute coronary syndrome and even acute ST-segment elevation myocardial infarction. Elevation of cardiac enzymes and abnormality of regional myocardial wall motion have been reported frequently for SAH. We report a case of an out-of-hospital cardiac arrest survivor with high suspicion of ST-...
متن کاملAngiographic Coronary Spasm in a Case of Spontaneous Subarachnoid Hemorrhage Mimicking Acute Myocardial Infarction
Neurologic stunned myocardium after subarachnoid hemorrhage (SAH) has been evidenced. Clinical presentations manifested as ST segment elevation by electrocardiography (ECG), left ventricular wall motion abnormality by echocardiography, and abnormal cardiac markers. The pathophysiology remains controversial. Coronary artery spasm has been proposed as a possible mechanism. However, most SAH patie...
متن کاملRegional myocardial perfusion after experimental subarachnoid hemorrhage.
BACKGROUND AND PURPOSE The pathophysiology of cardiac injury after subarachnoid hemorrhage (SAH) remains controversial. Data from animal models suggest that catecholamine-mediated injury is the most likely cause of cardiac injury after SAH. However, researchers also have proposed myocardial ischemia to be the underlying cause, as a result of coronary artery disease, coronary artery spasm, or hy...
